Like the other guys said, the answer is to meet the requirements, form kingdom by pressing the button in the top right cnr of the clan tab. Once you have formed your kingdom, talk to quest giver. If you talk about forming your own kingdom you can only say you are still working on it.

You can create them at Clan Tier 2 (I think), you just assign a companion in the Clan tab and he will start recruiting. I think mine is broken it says need a hero but I have 3. You need to hire a special companion. That's what they mean by "hero". From what I heard you can only control them if your in a faction.

It also helps a lot if you get your relationship up to about 60 with everyone you care about being friends with, before you leave the kingdom. I established a kingdom and selected Khuzait as my kingdom culture, but I still had a -3 Loyalty penalty in Baltakand. But the kingdom policies for choosing Khuzait were way better -- Grazing Rights (+.5 loyalty in each settlement) and Sacred Majesty (+3 influence/day) -- so I picked them. So check the policies you get and then choose.
Reminder, you need to be able to make your own kingdom to form parties, armies, polices etc. Taking over isn't enough, so keeping the Former Lord around for a while at least may be a good idea.
